How to access Out of Hospital Services

South Warwickshire NHS Foundation Trust has made a number of changes to the iSPA (integrated single point of access), to help you access Out of Hospital services.

A single phone number to make referrals

There is a single number you can ring to select the relevant service. This single contact replaces 13 individual numbers currently in operation.

Services covered are:

  • District nursing
  • Homefirst (CERT)
  • Speech and Language therapy
  • Dietetics
  • Podiatry and Orthotics
  • Physiotherapy and Occupational therapy
  • Community based specialist nursing – Parkinsons, Palliative Care, Diabetes, Heart Failure
  • Leg Ulcer Clinics
  • Continence service
