About this event
The Coventry and Warwickshire Integrated Care System (ICS) is introducing a new electronic information system to identify local groups of people who have chronic illness or are at risk of ill-health, so that health and care services can work together to improve the health outcomes of these groups.
This is known as Population Health Management.
